One of the first talents in the swordsmanship tree is Riposte:
Unlocks the ability to counter an opponent's blows.

I invested a point into it in my first playthrough, but I never really got it to work. I thought it was a passive effect of blocking, but that doesn't seem to be the case. It seems to be an active ability, but I haven't found out how to actually activate it.

How do I use riposte and counter-attack? Does it use up vigor?

From the manual:
You begin the counterattack with your block up (holding the E key) – when an opponent will try to land a blow on you, the icon on his body will turn into a sword icon for a moment, signifying the best opportunity to counterattack. At this point you should press LMB and Geralt will make a dodge and a dynamic riposte.
Yes, it uses up vigor as a normal block action.

Ok i have both the parry and riposte skills but i don't know how to use them, i hold block and thats all he does is block, and to be honest block doesn't even work half the time, half the time i hit block he doesn't raise his sword to block and i seem to have to wait or spam the key to get him to do it???

You need vigor or whatever it's called (the 'magic' energy') to parry. It's ... well, stamina basically

You have to hold down the block key ('E' on my keyboard), though sometimes I've also be able to time it right and hit the block key at the same time as an enemy was swinging at me and it blocked the strike. When you block a strike you also have to then re-hold the block key to get into blocking stance again. For parrying you hold block and wait until a sword icon comes up on the targeted enemy, if you click right when you see that (I always think I'm hitting too late because the icon doesn't stay on the enemy very long) Geralt will perform a parry move at that enemy. It works most of the time for me, though I don't know why the parry icon comes up sometimes and not other times.

for riposte you need to hold the block (default e) and when the enemy is about to attack the icon in his body should change to a sword icon for a moment, it is then that you click attack and Geralt should do the counter
